This form is for Jay’s constituents in the 8th Suffolk District to request assistance. You must be one of Jay’s constituents to access constituent services from Jay’s office. If you are not one of Jay’s constituents, you can easily find your state representative by looking up your address on Find My Legislator.
It is important that you fill out all the information asked for in this form so that we are able to:
Gather everything we need all at once so we can start processing your request faster
Verify that Jay is your state representative (or direct you to the correct office if you are represented by someone else)
Ensure that your information is up-to-date and that you are not being mistaken for another person in our records
Track the progress of your request in our system and make sure nothing slips through the cracks
